Whether you belong to Czech public health insurance is not decided by your passport. It is decided by your residence and your work. This page sets out who is in the system automatically, who has to buy commercial cover, what changed when the insurance monopoly ended, and why health insurance and income protection are two different things.
You are in public health insurance automatically if you hold permanent residence, or if you work for an employer based in the Czech Republic. For an employee the cover starts on the first day of work and ends on the last one. Anyone outside the public system who stays here longer than 90 days must hold comprehensive commercial health insurance.

In any month when the employment is not running, public cover stops and the gap has to be filled by a commercial policy. This is where most residence permit problems come from.
From 2021 only one insurer was allowed to sell comprehensive health insurance to foreigners. That monopoly ended on 20 September 2023 and you can now buy from any insurer licensed for this product. Price and scope differ between them, so comparing is worth the hour. Since 1 January 2024 children of foreigners under 18 also belong to public insurance by law, and a commercial policy cannot replace it.
Public cover starts on your first day and your employer registers you. Do not cancel the commercial policy before the registration is confirmed and before you know whether the immigration office still wants to see it.
Public cover ends on your last day of employment. From the next day you need another solution, usually a commercial policy if you do not hold permanent residence. A gap in cover is also a problem for the immigration authorities.
Yes, since September 2023 you can buy from any insurer licensed for this product. They differ in price, exclusions and payout limits, so comparing pays off.
They do different jobs. Health insurance pays for care, life and disability cover replaces income for your family. How much you need depends on your income, your obligations and what your employer already provides.
Health insurance premiums are not refunded, it is a pay as you go system. Pension insurance periods are a different matter. They are not erased and within the EU they are coordinated, so they still count later.
